Caractérisation pharmacogénétique des facteurs de résistance et de toxicité de l'Oxaliplatine et de l'Irinotécan dans les tumeurs colorectales

Abstract : Historically with a poor prognosis, colorectal cancer has been over the years the subject of many researches that have changed its supported. We mention here the evaluation of new therapies, the search for molecular predictors factors for chemotherapy sensitivity or even the search for new prognostic markers. In patients with metastatic colorectal cancer, the association of several cytotoxic agents into the chemotherapy treatment represents an important step in fighting this disease. It can lead to an increase in response rate and also an increase in patient survival. The protocols that use the association between 5-FU with folinic acid modulation and Irinotecan (FOLFIRI) or Oxaliplatin (FOLFOX) have become the most commonly used protocols in first-line chemotherapy for patients with metastatic colorectal cancers. Unfortunately, genetic polymorphisms in drug targets, or genetic polymorphisms in human drug-metabolizing enzymes as well as the genetic polymorphisms in genes involved in the process of DNA repair may influence the activity of the chemotherapeutic agents. Therefore we are interested in this thesis to identify genetics factors associated with toxicity and response to Irinotecan and Oxaliplatin in metastatic colorectal cancer. Individualizing therapy based on pharmacogenetic profile of each patient will make possible to obtain a lower toxicity and a higher efficacy of treatment, accompanied by an improved patients' quality of life.
